dc.description.abstractYöntemleri İnfeksiyöz mononükleoz, EBV’a bağlı olarak ortaya çıkan, primer olarak adolesan çağının ve gençlerin hastalığıdır. Hastalığın klasik triadı ateşin yanında boğaz ağrısı ve lenfadenopatidir. CMV ve EBV infeksiyonu daha çok çocukluk çağında asemptomatik olarak geçirilmekte iken, immun yetmezlik durumlarda ve konjenital infeksiyon gibi ağır klinik tablolara neden olmaktadır. Virusların kazanılmasında ve klinik infeksiyon şeklinde etkisi olan başlıca faktörler yaş, kalabalık ve kötü hijyen koşullarında yaşama, konağın immun durumu, kan ve organ nakli ve cinsel temastır. Çalışmamızda infeksiyöz mononükleoz tanısında güncel olarak kullanılan monospot deneyi ile ELISA ve Dot–Blot deneyi birlikte kullanılarak LAP ve boğazda şişlik şüphesiyle başvuran gruplardaki akut ve geçirilmiş infeksiyonlar araştırılmıştır. Çalışmaya, 18 ay süreyle hastanemize başvuran 2-59 yaşları arasındaki hastalar dahil edildi. Bu dönemde; 13’ü kadın, 14’ü erkek olmak üzere toplam 27 hastaya ait örnekler incelendi. Her bir hasta için hasta bilgilerini içeren bir form dolduruldu. Hasta serumlarında, EBV, (ELISA ile IgM, IgG, Monospot ve Dot-Blot yöntemi ile çalışıldı. CMV, ELISA yöntemi ile IgM, IgG ve Dot-Blot yöntemi ile çalışıldı. Çalışmaya alınan toplam 27 hastanın 5’inde (%18,51) EBV IgM (ELISA), 2 hastada (%7,40) Heterofil Antikor (Monospot), 4 hastada (%14,81) ise Dot Blot pozitif bulundu. CMV IgM ve IgG açısından değerlendirildiğinde; IgM ve Dot-Blot yöntemi hiç antikor saptanamazken, ELISA ile IgG 26 (%96,29) hastada pozitif olarak tespit edildi. Sonuç olarak, lenfodenopati ve boğazda şişlik şüphesiyle başvuran grupların infeksiyöz mononükleoz açısından değerlendirilmesinde en sık kullanılan yöntem olan monospot testinin tüm olguların saptamakta yeterli olmadığı görülmektedir. Hastalarda EBV ve CMV varlığının ELISA ile araştırılması yanı sıra, etkenleri ve heterofil antikor varlığını birarada değerlendiren Dot-Blot yönteminin pratik ve doğru sonuç veren bir test olarak kullanılabileceği düşünülmektedir.en_US
dc.description.abstractInfectious mononucleosis, is a primarily disease of the adolescence and childhood period caused by EBV. The classical triad of the disease is sore throat and lymphadenopathy besides fever. While infections of CMV and EBV are exposured mostly in childhood period, they can cause serious clinical pictures in immune deficiency statuses or congenital infections. The main factors effecting the acquiring of viruses and clinical infections are depended on age, living in a crowded environment, and bad hygienic conditions, the immune status of the host, blood and organ transplantation, and sexual intercourse. In our study, we investigated acute and past infections in the patients admitted with lymphadenopathy and doubtful swelling in throat by using monospot test, which is a test used in the diagnosing of infectious mononucleosis currently, and ELISA and Dot–Blot tests together. Patients of ages between 2 and 59 who applied to our hospital in a period of 18 months were involved in our study. During this period, samples taken from a total of 27 patients, out of which 13 were females, and 14 were males. A form containing the information about patients were filled for each patient. EBV was searched in the serum with ELISA for IgM and IgG were searched, and also with Monospot and Dot-Blot methods. CMV was searched, with ELISA method, for IgM and IgG, and also were searched with Dot-Blot method. In 5 (%18,51) of the patients out of 27 patients, EBV IgM ELISA was found to be positive, in 2 (%7,40) heterophile antibody (Monospot), and in 4 (%14,81) Dot-Blot was found to be positive. When results were evaluated in terms of CMV IgM and IgG, while no antibodies could be found with ELISA and Dot-Blot methods, antibody was determined with (ELISA) IgG in 26 (%96,29) patients. In conclusion, it is apparent that the monospot test, which is the most frequently used method in the evaluation of infectious mononucleosis, is not satisfactory in evaluating every case of in such patients with lymphadenopathy and swelling in throat. It is suggested that Dot-Blot method is a practical and accurate test that is another method in evaluating the factors and the presence of heterophile antibodies, as well as searching the presence of EBV and CMV with ELISA.en_US
