Overin müsinöz kistadenokarsinomları ve sassone skorlama sistemi

Abstract

Amaç: Müsinöz hücreli over kanserli hastaların preoperatif ultrasonografik bulguları, malign olguların klinik tanısında kullanılan Sassone Skorlama Sistemine göre değerlendirilerek intraoperatif cerrahi evreleme ile korelasyon araştırıldı. Materyal ve Metod: Müsinöz hücreli over kanseri tanısı almış 27 olgu yaşları, menopozal durumları, önceki operasyonları, asit varlığı, ultrasonografik bulguları, preoperatif CA-125 düzeyleri, tümörün büyüklüğü, ultrasonografik görünümü, intraoperatif evreleme (FIGO sınıflaması) açısından değerlendirildi. Ultrasonografik bulgular Sassone Skorlama Sistemine göre değerlendirildi. Skor sonuçları ve CA-125 değerleri ile tümör evrelerinin korelasyonu karşılaştırıldı. Bulgular: Hastaların ortalama yaşı 52.4 (21-76) idi ve 13 hasta tanı anında premenopozal dönemdeydi. 7 (%26) olguda tümör bilateral idi. Ortalama tümör çapı 20 cm (5-30 cm) idi. Hastaların 9 (%33)’unda tanı anında asit mevcuttu. Ultrasonografik bulgular için Sassone Skorlaması uygulandığında en yüksek değerler evre IA’da ve en düşük değerler evre IC’de bulundu. En düşük CA-125 değerleri evre IA’da ve en yüksek değerler evre IIIC’ de izlendi. Çalışmamızda over musinöz kistadenokarsinomlarının intraoperatif evreleri ile Sassone Skorlama Sisitemi arasında bir korelasyon izlenmezken tümör evresi ile preoperatif CA-125 düzeyleri arasında pozitif korelasyon tespit edildi. Sonuç: Sassone Skorlama Sisteminin over musinöz kistadenokarsinomlarının intraoperatif evreleri ile uyumlu olmadığı sonucuna ulaşıldı.

Objective: Preoperative ultrasonographic findings of ovarian mucinous cystadenocarcinoma cases were evaluated due to Sassone’s Scoring System and the correlation between the scores and clinical stages was investigated. Material and Methods: 27 cases of mucinous cystadenocarcinoma were evaluated due to their menopausal status, presence of ascite, ultrasonographic findings, preoperative CA-125 levels, size of tumor, intraoperative staging (FIGO classification). Ultrasonographic findings were evaluated with Sassone’s Scoring System and correlation between these findings and CA-125 levels and stages of the tumor was investigated. Results: The average age of the cases was 52.4 (21-76). 13 cases (48.1%) were in premenopausal period at the time of diagnosis. Preoperative average tumor diameter was 20 cm (5-30 cm) and in 7 cases (26%) tumors were bilateral. 9 of 27 cases (33%), had ascittes at the time of diagnosis. When Sassone Scoring System for evalution of ultrasonographic findings was used, the highest scores were in stage IA and the lowest scores were in stage IC. The lowest CA-125 levels were in stage IA and the highest CA-125 levels were in stage IIIC cases. In our study we did not find any correlation between intraoperative stages of ovarian mucinous cyctadenocarcinoma and scores of Sassone Scoring System, but there was positive correlation between preoperative CA-125 levels and stages of tumors. Conclusion: We concluded that there was no correlation between intraoperative stages of ovarian mucinous cystadenocarcinoma and scores of Sassone scoring system.
