Kütahya Devlet Hastanesi Genel Cerrahi Kliniğinde Ameliyat Edilen Hastaların Ameliyat Öncesi Eğitimi ve Bu Eğitimi Alma Düzeylerinin İncelenmesi

Abstract

Bu araştırma; cerrahi hastaları için son derece değerli bir kriter olan ameliyat öncesi eğitimin öneminin vurgulanması ve operasyon uygulanmış hastanın ameliyat öncesi eğitim alma düzeyinin saptanması amacıyla tanımlayıcı olarak planlanmıştır. Araştırma Kütahya Devlet Hastanesi genel cerrahi kliniğinde yatmakta olan operasyon uygulanmış 100 hasta üzerinde gerçekleştirilmiştir. Bilgi toplama aracı olarak hazırlanan anket formu, araştırmacının kendisi tarafından hastalarla yüz yüze görüşülerek uygulanmıştır. Verilerin analizinde, yüzdelik hesapları ve ki-kare önemlilik testi kullanılmıştır. Verilerin değerlendirilmesi sonucunda cerrahi servis hemşirelerinin ameliyat öncesi devrede hastaları yeterince bilgilendirmedikleri, hastaya ameliyat öncesi devrede verilmesi gereken eğitimin öneminin yeterince bilincinde olmadıkları ve hastaların büyük çoğunluğunun ameliyat öncesi eğitim almadığı saptanmıştır. Cerrahi servis hemşirelerinin ameliyat öncesi eğitim içeriğinde yer alan hastaların ameliyat sonrası uygulaması gereken egzersizler konusunda da hastaları yeterince bilgilendirmedikleri, bu egzersizleri ameliyat öncesi devrede servis hemşirelerinden öğrenen hastaların oldukça düşük oranda olduğu saptanmıştır. Hastalara ameliyat öncesi devrede cerrahi servis hemşireleri tarafından verilen eğitimin; içerik, verildiği zaman ve sıklık açısından istenilen nitelikte olmadığı; servis hemşirelerinin öğretmeleri gereken egzersizler konusunda daha çok yatak-içi dönme ve ekstremite egzersizlerini dikkate alıp verdikleri eğitimde sadece bu egzersizlere ve ameliyat sonrası erken mobilizasyon konularına yoğunlaştıkları, ameliyat öncesi eğitim için çoğu zaman yanlış zamanı seçtikleri ve bu eğitimi uygun sıklıkta vermedikleri saptanmıştır. Ameliyat öncesi eğitim alan hastaların büyük çoğunluğunu öğrenim düzeyi yüksek hastaların oluşturduğu, hastaların öğrenim düzeyleri yükseldikçe ameliyat öncesi eğitim alma durumunun da arttığı saptanmıştır. Ameliyat sonrası hareketsizlik yada uygulanması gereken egzersizler hakkında bilgisini ifade eden hastaların çoğunluğunun bilgi kaynağının çevredeki insanlar olduğu, bilgi kaynağı cerrahi servis hemşireleri olan hastaların daha azınlıkta olduğu ve bu hastaları lise ve üniversite mezunu hastaların oluşturduğu saptanmıştır. Hastaların büyük çoğunluğunun cerrahi servis hemşirelerinden memnun oldukları, onlardan hiçbir beklentilerinin olmadığı, memnun olmayanların ise anlayış ve açıklama beklediği ve bu beklentisini ifade eden hastaların çoğunluğunun ilkokul mezunu olduğu saptanmıştır.

This research was planned to emphasize the importance of pre-operational training which is of the top necessity, and it was designed as a descriptive method so as to determine how much pre-operational training a patient who had already had an operation received. The above mentioned study was carried out on 100 patients who had had operations and then were taken care of at the General Surgery Department of Kutahya State Hospital. The questionnaire which was designed in order to collect relevant information was applied to the patients by the researcher interviewing with the patients face to face. In analyzing the data, percentage calculations and Ki-square necessity test were made use of. Upon evaluating the data, it was discovered that the patients weren’t informed sufficiently about the required information by the surgery service nurses in the pre-operational period, and that the service nurses weren’t conscious of the importance of the process of pre-operational training of the patients, and consequently, the majority of the patients weren’t aware any of the pre-operational procedures. It was found out that the surgery service nurses didn’t inform the patients sufficiently about the necessary after-operation exercises while giving pre-operational training to them, and that the patients who had learned about these exercises form the service nurses were quite low in percentage. It was also monitored that the training provided by the surgery service nurses, in the pre-operational period, wasn’t of the required quality regarding the content, timing, frequency; and that these service nurses only focused on in-bed rotation and extremity exercises rather than other relevant exercises they should have given; and that neither their timing nor their frequency of exercises was required. It was observed that a great number of the patients who had received pre-operational training consisted of people whose educational levels were high, and that their number was increasing as their educational level was rising. Another striking finding was that the informed patients were mostly informed about the exercises by other patients or by the people around. Furthermore, the informed patients who had got the information from the surgery service nurses were the minority and their educational levels were either high school or university graduation. Finally, it was also found out that the majority of the patients were pleased with their surgery service nurses, and they didn’t have any expectations from their service nurses, and yet, the unsatisfied ones were primary school graduates who were expecting sympathy and more explanation about the routines.
