dc.description.abstractİşitme, dış ortamda bulunan ses dalgalarının dış kulak ve orta kulak yolu üzerinden iç kulağa aktarılması sonucu oluşan işaretlerin sinir hücreleri ile işitme korteksine taşınması olayıdır. Dış, orta, iç kulak veya akustik sinirde meydana gelen rahatsızlıklar nedeniyle çevredeki seslerin algılanamaması işitme kaybı olarak adlandırılmaktadır. İşitme kayıpları iletim tipi, sensörinöral tip ve miks tip olarak üç kategoride sınıflandırılmaktadır. Saf ses odyometri testleri ile işitme kaybının tipi ve derecesi belirlenebilmektedir. Bu çalışma odyometri uzmanına gerek duymadan okullarda, aile sağlık merkezi ve halk sağlığı merkezlerinde ön teşhis koymak için yapay zekâ destekli bir sistemin geliştirilmesini amaçlamaktadır. Farklı desibel ve frekans değerlerindeki saf ses ve maskeli sesler yazılım aracılığı ile kişiye uygulanarak hava ve kemik yolu testi yapılabilmektedir. Geliştirilen sistemin arayüz platformu Visual Studio C# programında hazırlanmış ve eğitimi tamamlanmış olan yapay sinir ağının ağırlık katsayıları programa entegre edilmiştir. Test sonucunda elde edilen veriler yapay zekâ algoritması ile analiz edilerek işitme kaybının tipi ve derecesi belirlenmiştir. Yapay zekâ algoritmasının eğitimi için toplanan 200 adet verinin %70’i öğrenme, %15’i geçerlilik, %15’i test verisi olarak kullanılmıştır. Sistemin güvenirliliği yapay sinir ağına gösterilmeyen 48 adet veri ile test edilmiştir. 48 adet test verisinin sınıflandırılması öncelikle Afyon Devlet Hastanesi’nde görev yapan Kulak Burun Boğaz uzmanı tarafından daha sonra gerçekleştirilen sistem ile sınıflandırılmıştır. Bu veriler ikili sınıflandırma yöntemi ile analiz edildiğinde mikst tip işitme kayıpları ve sağlıklı bireyler arasında elde edilen sonuçlarda hassasiyet %92,3, doğruluk oranı %95,8, negatif prediktif değer %91,6 olarak bulunmuştur. İletim tipi ve sensörinöral tip işitme kayıplarının sağlıklı bireylerle kıyaslanmasından elde edilen hassasiyet, doğruluk ve negatif prediktif değer ise %100 olarak bulunmuştur. Elde edilen bulgular geliştirilen sistemin işitme kaybının tipini ve derecesini sınıflandırmada ve ön tanı koymada etkin olduğunu göstermektedir. Ancak bu tarz sistemlerin sağlık alanında hekimlerin yerine kesin tanı koymak için kullanılması düşünülmemelidir. Çünkü işitme kaybı hastalıkların altında yatan farklı fizyolojik veya psikolojik alt etmen bulunabildiği düşünülerek son kararın uzman tarafından verilmesi beklenmelidir. Sonuç olarak bu sistemin hastalıkların sınıflandırılmasında destekleyici sistem ve yazılımlar olarak tanımlanması daha doğru bir yaklaşım olacaktır.en_US
dc.description.abstractHearing is the event of the transmission of the signals formed as a result of the transfer of sound waves in the outer environment to the inner ear through the outer and middle ear canal to the auditory cortex by nerve cells. Inability to perceive surrounding sounds due to disturbances in the outer, middle, inner ear or acoustic nerve is called hearing loss. Hearing losses are classified into three categories as conductive type, sensorineural type and mixed type. With pure tone audiometry tests, the type and degree of hearing loss can be determined. This study aims to develop an artificial intelligence-supported system for pre-diagnosis in schools, family health centers and public health centers without the need for an audiometry specialist. Pure sound and masked sounds at different decibel and frequency values can be applied to the person through the software and air and bone conduction tests can be performed. The interface platform of the developed system was prepared in the Visual Studio C# program and the weight coefficients of the artificial neural network, whose training was completed, were integrated into the program. The type and degree of hearing loss were determined by analyzing the data obtained as a result of the test with an artificial intelligence algorithm. Of the 200 data collected for the training of the artificial intelligence algorithm, 70% was used as learning, 15% as validity and 15% as test data. The reliability of the system was tested with 48 data that were not shown to the artificial neural network. Classification of 48 test data was firstly classified with the system implemented later by the Otorhinolaryngologist working at Afyon State Hospital. When these data were analyzed with the dual classification method, the sensitivity was 92,3%, the accuracy rate was 95,8%, and the negative predictive value was 91,6% in the results obtained between mixed hearing losses and healthy individuals. The sensitivity, accuracy and negative predictive value obtained by comparing conductive and sensorineural hearing losses with healthy individuals were found to be 100%. The findings obtained show that the developed system is effective in classifying the type and degree of hearing loss and in making a pre-diagnosis. However, such systems should not be considered to be used to make a definitive diagnosis for physicians in the field of health. Because, considering that there may be different physiological or psychological factors underlying hearing loss diseases, the final decision should be expected by the specialist. As a result, it would be a more correct approach to define this system as supportive systems and software in the classification of diseases.en_US
